We want our providers to focus on patient care. To support our providers and their staff, we are committed to doing what we can to minimize administrative activities through the deployment of technology, simplified processes and dedicated personnel to make United Physicians International easy to do business with.

Provider Portal

We offer a web-based provider portal for our provider partners and their office staff. Authorization requests, claims submissions and status updates are available electronically through the provider portal.

If you do not have a user account for the provider portal and would like to have one set up, please complete and return the MSOExec Web Portal Access Request Form and Agreement via fax to (626) 658-2833.


We are committed to complying with all federal, state, local laws and contractual obligations to our health plans. By completing the Centers for Medicaid and Medicare Services (CMS) compliance trainings, reviewing our Code of Conduct and signing an attestation, as a provider partner, you are agreeing to uphold yourself to the same standards as United Physicians International, Inc.

All providers are required to sign and submit a compliance attestation to United Physicians International, Inc. on an annual basis.

Contracting & Provider Relations

Joining United Physicians International, Inc.

Providers interested in becoming part of the United Physicians International network should fax the following information to the Contracting Department at (626) 458-8051:

  • Letter of Interest - please include scope of services, hospital/facility privileges
  • Curriculum Vitae
  • W-9
We will respond to all interested providers within thirty (30) days.
